How to Write ICU Shift Reassessment Note

Reassessments completed. No acute events has occurred since the last assessment. No changes noted since the last assessment. Neurologically intact. Cardiac rhythm unchanged. Blood pressure stable. All vital signs noted. Respirations regular and unlabored. Denies chest pain or discomfort. Active bowel sounds noted on all quadrants. Adequate urine output noted. Turned to the right side for comfort and skin care. Educated on rationale of frequent turning for skin care. Encouraged to verbalize needs and feelings. Denies any pain at this time. Call button in reach. Bed low. See flowsheet for drips rate. Will continue monitoring
